Should you wear your PFD all the time, and should you as the skipper of your vessel ask / require your occupants to do the same regardless of if you're underway or not? Typicall when things go south in a boat, they do so in a hurry chaotic fashion.

YES, AS MIKE STATED...I DO DEMAND THAT (WHILE UNDERWAY) ALL OF MY (occasional) PASSENGERS WEAR A PFD AND MYSELF! I have two of the high-end PFD's (HIT-hydrostatic inflation technology devices) worn like a very, very light weight vest. Daughters bought my first one due my development of vertigo (three years ago)...I fish 90% of the time alone. Since then purchased a second for the 'queen-bee' to wear...soon to purchase a third (two up and coming grandson fisherpersons). The HIT's deploy ONLY AFTER an atmospheric-pressure change (2" - 3" below water surface)...HIT WILL NOT DEPLOY after being soaked by sea-spray, hosing down, constant damp storage, etc...)! Spot on Big John. As the skipper of your vessel you are ultimately responsioble for the safety of those on board regardless of their skill level and as you know I am an accomplished and strong swimmer in most any age group, but I still wear mine when I fish because it's the responsible thing to do. Anyone who doubts my ability is welcome to join me as my quest at the swim culb for laps to see how it plays out. Looser buys a beer afterwards,
